The data for the father questionnaire are listed in the table father.
     The table is provided as SAS and SPSS-file.                                                           

     A documentation of all variables is given as an additional Excel-file

     An annotated CRF is also included (variable names were added in red and coding in  blue
     colour onto the original questionnaire).
